From The Heart (FTH) is a Georgia for-profit corporation formed and incorporated in October’ 2000. From the Heart is a community based Personal Care Home / Day Support Program Agency that provides supports and meets the needs of individuals with Intellectual and Developmental Disabilities and their families/care givers.
To be happy, people need to feel like they have some control of what happens to them, like making choices, sharing places, and developing relationships. FTH offers individuals with special needs an opportunity to live as independently as possible, develop relationships, and live in their community with security and support. Since 2006, From the Heart, was awarded and maintains a CARF Accreditation for Community Service and Community Housing.
Quality improvement represents a practical approach to achieving optimal outcomes for individuals and their families/care givers. FTH leadership drives and endorses change to ensure that the organization’s vision and mission are aligned with the needs and expectations of the people we serve The management demands a practical method for continuous improvement that seamlessly integrates all organizational functions and the engaged input of all stakeholders and to ensure that organizational purpose, planning, and activity result in the desired outcomes. The personal goal and aspiration of the President of From The Heart is “that anyone that comes through the doors of From the Heart, leaves a better, more productive person!”

We accomplish this by ensuring that the individual services are person-centered and that the planning and delivery of our services address what is important to and for the individuals. Based on capacity and capabilities of the Individual safeguards are in place so that satisfactory individualized outcomes are recorded.
